When a single email field is not enough, the Form Gatherer lets you build a custom form with multiple fields — perfect for quotes, registrations, surveys or detailed lead capture.

Build your form

Add the fields you need, each with its own label and type:
Text — names, short answers
Email — validated email address
Number — quantities, ages, etc.
Phone — contact numbers
Textarea — longer messages

Mark any field as required so visitors cannot submit without filling it in.

Where the data goes

Every submission is stored under the notification and can be forwarded from the Data tab — to a webhook, your email, or a CRM via Transmit Code. The values can also be shown in other notifications using dynamic variables.

Tip: keep forms short. Every extra field lowers completion, so only ask for what you truly need.
